Does your complaint or condition affect your daily life? With a ‘Gezondheidsroute’, we work with you to identify your needs, bring together the right support and help you regain control of your daily life.
Enjoy life more
For example, you may have rheumatism, diabetes, chronic pain or mental health issues. Your GP or specialist will help you with your medical treatment. That care remains the foundation. But a diagnosis does not tell the whole story about how you feel and what you are able to do in your daily life.
Exercise, sleep, diet, stress, energy levels, work and social activities can also have a significant impact. During a medical consultation, there isn’t always enough time to consider these matters in detail. Help is often available, but it is spread across various healthcare providers and organisations. This can make it difficult to know where to start.
That is why Rondom has developed the Health Pathways. Various healthcare providers work together within Rondom. A case manager will work with you to identify what is important and what support is appropriate for your situation.
Sometimes, a single healthcare provider is enough. Sometimes, several healthcare providers work together. In that case, you don’t have to organise everything yourself or keep repeating your story. This is how we help you to live as healthily as possible and enjoy life more.

Find the route that suits you
Rondom has five Routes. Each Route focuses on a different condition or set of symptoms, but is based on the same personalised and comprehensive approach. The Rheumatism Route is the first to be available. The other Routes will follow shortly.
Rheumatism Route
For people with rheumatism who, alongside their medical treatment, are seeking help with mobility, fatigue, work and everyday life
Chronic Pain Pathway
For people with chronic pain who want to regain more control over their mobility, energy levels and daily life
Mental Health Challenges Route
For people experiencing stress, anxiety or low mood who want to regain control and find balance in their daily lives
Diabetes Route
For people with diabetes who, alongside their medical treatment, are seeking support with exercise, emotional wellbeing, lifestyle, work and everyday life
Oncology Route
For people living with or after cancer who, alongside their medical treatment, are seeking support with energy levels, exercise, work and daily life
Our approach
Your body, how you feel, your lifestyle and your daily habits often influence one another. For example, poor sleep can affect your energy levels. Pain or uncertainty can lead to you being less active. And when it becomes harder to move around, work and daily activities can become more of a struggle.
That is why, within a Route, we look at these areas in conjunction with one another. That does not mean you need help with everything. Your Route may start with a physical complaint, how you are feeling, or a question about your lifestyle or daily life. Sometimes help in one area is enough. Sometimes a combination works better.

Who are the Routes for?
A Health Programme may be suitable for you if a condition or persistent symptom is affecting your daily life. You may already be receiving treatment from a GP or specialist, but still have questions that are not fully addressed during your medical treatment.
A Route may be right for you if you:
- is looking for help that looks beyond just your symptoms or diagnosis;
- are unsure which healthcare provider to start with;
- in addition to your medical treatment, you are seeking help with exercise, energy, sleep, stress, feet, work, aids or daily activities;
- notices that help from several healthcare providers may be needed;
- someone you can turn to who will help you keep track of things.
Case manager
As part of your Route, you will be assigned a case manager. This person will be your dedicated point of contact throughout the Route. The case manager is familiar with your situation and will help you to receive the right support.
The case manager:
- listens to your story and discusses what you want to achieve;
- helps you decide where best to start;
- checks which healthcare provider is right for you at that moment;
- works with you to keep track of things;
- will consult, where necessary and with your consent, with your GP, medical specialist and care providers;
- regularly discusses with you how you are getting on;
- will work with you to adjust the plan if your situation changes;
- helps you find alternative care if necessary;
- will work with you to look at reimbursement, insurance and any potential costs.
How often you are in contact with your case manager varies from person to person. Together, you will agree on how much contact you need and what suits you best. You always have a say in your goals, the support you receive and the pace of the Route.
This way, you won’t have to figure out for yourself who to turn to; you’ll have someone who thinks things through with you, and care will be taken to ensure that the various aspects of your care are well coordinated.

Support, even between appointments
Change happens in everyday life. That is why, within a Route, you can also use the Rondom app alongside the practical support provided.
In the app, for example, you can:
- view your appointments and personal goals;
- find information, exercises and practical tips;
- view relevant information and exercises that your practitioner has prepared for you;
- completing questionnaires;
- easily send messages to your case manager and care providers.
This way, you have all the information in one place, you can continue working on your goals at home, and you can easily stay in touch with your healthcare providers. The app complements your face-to-face appointments.
