Questionnaires

We use questionnaires. These help us gain a clear picture of your symptoms, health and situation before your treatment begins. The questionnaires also help us to find out how you are getting on, both during and after your treatment. This enables us to tailor your care more effectively to your needs and leaves more time during your appointment.

From time to time, we may ask you to complete a questionnaire. Below you can find out which questionnaires these might be. Completing the questionnaires is voluntary and takes just a few minutes.

1. A general health questionnaire to complete before your first appointment: In this questionnaire, we take a broad look at your health and daily life. The questions cover, for example, how you feel, how much you exercise and what is important to you. This helps us gain a complete picture of your health.
2. A questionnaire to help you prepare for your first appointment: This questionnaire focuses specifically on your symptoms and the reason for your appointment. This will enable us to tailor your care more effectively to your needs. It will also leave more time for your examination and treatment.
3. 3 weeks after your first appointment: a questionnaire about how you felt the appointment went In this questionnaire, we are asking how you found the survey and your initial contact with us.
4. Once your treatment is complete: a questionnaire about how you are getting on now In this questionnaire, we are asking how you are getting on at the moment and whether your symptoms have improved. This will give us a good idea of the outcome of your treatment.

Rondom Foundation

For every completed questionnaire, we donate a sum to our Rondom Foundation. Through this foundation, we help people who need care but are unable to pay for it (in full). In this way, we work together to ensure that our care remains accessible to everyone.

Scientific research

We can also use the results of completed questionnaires for scientific research. This enables us to further improve our occupational therapy services. It helps us to provide a scientific basis for our care.

We store your answers securely. Only the healthcare professionals treating you can view certain answers. We use the answers anonymously or in coded form for research and quality improvement purposes.
