SLIMMER programme
Living a healthier life often starts with small changes. But it can be difficult to stick to these healthy changes in everyday life. The SLIMMER programme helps you work towards a lifestyle that suits you.
During the programme, you will receive guidance from various healthcare professionals who will support you in making healthy choices. For example, in the areas of diet, exercise, relaxation and developing new habits. In this way, you will work towards a lifestyle that is achievable and that you can maintain in the long term.
Please note: The SLIMMER programme is only offered in Gorinchem.

GLI programme
The SLIMMER programme is one of the recognised programmes within the Combined Lifestyle Intervention (GLI). A GLI is a programme that supports people in developing and maintaining a healthier lifestyle in order to reduce excess weight or minimise health risks.
Within a GLI, various lifestyle themes are addressed. These include diet, exercise, behaviour, sleep, relaxation, relapse and sustainable behavioural change. The aim is not only to live a healthier life, but above all to maintain these changes in a way that is achievable in everyday life.
A GLI programme always lasts two years and begins with an intake interview. During this interview, we will discuss together whether the programme is suited to your situation, goals and motivation.
What do we offer?
The SLIMMER programme is a recognised lifestyle programme within the GLI. During the programme, you will receive guidance from a lifestyle coach, physiotherapist and a dietitian. Together, they will guide you in developing a healthier lifestyle.
The programme consists of group sessions and one-to-one consultations. It also provides an opportunity to apply lifestyle topics to your own situation and daily habits.
The SLIMMER programme is a coaching programme, not a sports programme. You exercise on your own initiative. During sessions, however, short exercise routines are used to help you experience how exercise can become part of your day in an accessible way.
During the programme, we will be working on, amongst other things:
- gain an insight into your lifestyle and daily habits;
- healthy and appropriate eating habits;
- get more exercise in a way that suits you;
- coping with stress and relaxation;
- sleep better and feel more energetic;
- behavioural change and maintaining new habits;
- working towards goals that fit in with your everyday life.
At present, the SLIMMER programme is only available in Gorinchem.

What can you expect?
The SLIMMER programme lasts two years and consists of three phases.
Phase 1: intake
The programme begins with an initial consultation. Together, we will discuss your goals, motivation and lifestyle.
Phase 2: the first six months
The first six months are more intensive. You will have a weekly session lasting approximately 60 minutes. During these sessions, various lifestyle topics will be discussed. There will also be a focus on exercise in an accessible way.
Between sessions, you’ll be working on your own. You’ll read information, work on action points and discover which changes suit your daily life.
Phase 3: maintenance
After the first six months, the maintenance programme begins. During this phase, you will continue to work on your lifestyle in an increasingly independent manner. You will have a few more sessions with your lifestyle coach and meet your group at follow-up meetings.
Furthermore, you can always count on:
- care provided by several healthcare professionals;
- focus on your personal goals;
- support in developing new habits;
- practical tips for at home;
- guidance aimed at lasting change.
The aim of the SLIMMER programme is to help you make changes and maintain them in the long term.
Collaboration with other healthcare providers
As part of the SLIMMER programme, you will work with various healthcare professionals. The lifestyle coach will support you with behavioural change and lifestyle. The dietitian will guide you on nutrition and eating habits. The physiotherapist will help you to incorporate exercise into your daily life in a way that suits you.
By working together, support and advice are well aligned.
