Tarsal tunnel syndrome

Do you have a burning pain, tingling or numbness on the inside of your ankle, under your foot or in your toes? If so, you may be suffering from tarsal tunnel syndrome.

The symptoms may worsen when standing for long periods, walking or exercising. Sometimes the symptoms are mainly present in the evening or at night.

What is it?

In tarsal tunnel syndrome, a nerve on the inside of the ankle becomes trapped. This nerve runs through a narrow space behind the ankle. We call this space the tarsal tunnel.

If the space in this tunnel becomes narrower or if there is increased pressure on the nerve, the nerve may become irritated. This can cause pain, tingling or a numb sensation in the foot.

There is not always a single clear cause of tarsal tunnel syndrome. This condition often arises as a result of pressure being exerted on a nerve.

Other factors can also contribute to the development of tarsal tunnel syndrome, such as:

  • an abnormal position of the foot;
  • swelling following an ankle sprain;
  • tendon irritation;
  • scar tissue;
  • overload;
  • shoes that are too tight;
  • a ganglion (a fluid-filled swelling).
What do you notice?

Symptoms can vary from person to person. They often involve nerve-related symptoms on the inside of the ankle and under the foot.

You may notice that:

  • if you experience a burning, tingling or numb sensation;
  • the pain radiates to the sole of the foot or the toes;
  • Standing or walking for long periods makes the symptoms worse;
  • exercise causes complaints;
  • the symptoms get worse in the evening or at night;
  • rest, elevating the leg or massaging it provides temporary relief.
The impact of a healthy lifestyle and your mental health

A healthy lifestyle can have a positive effect on the symptoms of tarsal tunnel syndrome. Getting enough exercise within your capabilities can help to keep your foot and ankle supple. Maintaining a healthy body weight, wearing good shoes and allowing sufficient time for recovery can also help to reduce pressure on the nerve.

Mental health also plays a part. Stress, poor sleep or excessive worry can make pain and tingling feel more intense. This can be particularly troublesome in the case of nerve-related symptoms, as burning pain or tingling demand a great deal of attention.

What can you do yourself?

You can try to reduce the pressure on the inside of your ankle and foot yourself.

To reduce symptoms, you can:

  • wear shoes that don’t pinch;
  • temporarily restricting standing or walking for long periods;
  • gradually build up your exercise regime;
  • adjust your activities if tingling increases;
  • allow the foot sufficient rest if it is painful;
  • striving for a healthy weight;
  • Do not leave it too long if you have nervous symptoms that are getting worse.
Advice and treatment

Our podiatrists They will examine your foot position, the strain on your foot and ankle, and the way you walk. The aim is to reduce the pressure on the nerve.

Treatment may consist of:

  • podiatric insoles;
  • shoe advice;
  • tape to provide temporary support;
  • advice on stress and recovery;
  • ultrasound scan;
  • collaboration with a physiotherapist.

If your symptoms persist, worsen or you notice a loss of strength, further medical investigation may be required to investigate the cause of your symptoms. In that case, we will refer you back to your GP.

Shoe advice

If you have a misaligned big toe, it is important to bear this in mind when buying new shoes. Please note the following:

  • sufficient length and width, so that the toes have room. Shoes that are too narrow actually put extra pressure on the big toe;
  • Buy shoes at the end of the day, as your feet are often slightly swollen then. That way, you can be sure the shoes fit properly;
  • shoes made from soft and supple materials. Soft leather or supple fabric moulds better to the foot. This puts less pressure on the bump near the big toe;
  • a good flex point on the shoe. The shoe must bend at the same point as your forefoot.
