Plantar fasciitis

Your heel hurts as soon as you get out of bed or after you’ve been sitting for a while. The pain may subside at first when you walk, but the symptoms may return later in the day. This can make standing for long periods, walking or exercising increasingly difficult.

Tendinitis often develops gradually. At first, you might carry on as normal, hoping it will go away on its own. But when the symptoms keep coming back, it can be frustrating – especially when moving around or carrying out daily activities becomes more of a struggle.

Plantar fasciitis is also often referred to as fasciitis or heel spur.

What is it?

The plantar fascia is located under your foot. This is a strong band of tissue that runs from the heel to the forefoot. The plantar fascia supports the arch of the foot and helps with propulsion whilst walking.

Repeated strain can cause the plantar fascia to become irritated or overloaded. This condition is also known as fasciitis or heel spur. However, a heel spur is not always the cause of the pain. A heel spur is a bony protrusion on the heel bone that sometimes occurs with these symptoms, but not everyone with heel pain has a heel spur.

Tendinitis usually results from overuse of the tendon plate. This means that more strain is placed on the tendon plate than the foot can cope with.

Other factors can also contribute to the onset of symptoms, such as:

  • an abnormal position of the foot;
  • a different way of walking;
  • shoes that do not fit properly or provide insufficient support;
  • excess weight;
  • prolonged strain on the foot.
What do you notice?

Symptoms of tendinitis often develop gradually. At first, you will mainly notice the pain when moving. If the symptoms persist, you may also feel pain after a period of rest.

You may notice that:

  • walking or standing is painful;
  • you feel pain under your heel;
  • the first steps in the morning are painful;
  • the foot feels stiff;
  • standing or walking for long periods causes symptoms;
  • Standing on tiptoes or bending your toes is painful.

If you have tendonitis, it is important to keep moving, but only within the limits of the pain. Alternate walking, standing and exercising with plenty of rest. This will prevent the tendon from becoming overloaded again and again.

Sufficient sleep, a healthy diet and a healthy body weight support recovery. Being overweight can put extra strain on the feet. It is also important to gradually build up walking, exercise and other activities.

Long-term health issues can be frustrating or cause anxiety. Sometimes this leads you to move less, or conversely to carry on walking for too long despite the pain. A clear plan helps you to gradually build up your activity levels. What can you do yourself?

There is a lot you can do yourself to alleviate the symptoms of tendinitis. It is important to temporarily reduce the strain on your foot and to get plenty of rest.

To reduce symptoms, you can:

  • temporarily reduce the time spent standing or walking;
  • gradually build up exercise and sport;
  • wear shoes with sufficient support and cushioning;
  • Do not walk barefoot on hard surfaces;
  • get plenty of rest;
  • adjust your activities if the pain increases;
  • striving for a healthy weight.
Advice and treatment

Tendinitis can be a long-term condition. Symptoms often persist for a considerable period. Early treatment can help to speed up recovery.

Treatment may consist of:

  • podiatry insoles to distribute pressure more evenly;
  • footwear advice for greater support and stability;
  • tape to temporarily relieve pressure on the tendon plate;
  • exercises to make the foot and calf muscles stronger and more flexible;
  • ultrasound examination;
  • dry needling;
  • advice on stress and recovery.
